Insurance Call Script

The purpose of this script is to give you guidance in negotiating the insurance process and ensure that our time together is covered by your insurance plan.

Call the member services number on the back of your card and ask:
 

1. Does my plan cover outpatient nutrition counseling (Procedure Codes 97802 and 97803)? Y / N

   • If yes, how many sessions are allowed?

   • Are there any restricted diagnoses? (ask specifically about the diagnosis code Z71.3 and if it’s covered as preventive)
      

2. Do I have a deductible to meet before insurance pays? Y / N

   • If yes, how much?
      

3. Will I have a copay or co-insurance? Y / N

   • If yes, what will it be?
      

4. Do I need a physician referral? Y / N

   • Note: if you need a physician referral this must be done at least 1 week prior to our session.
      

5. Is Numena Nutrition in-network (NPI 1770193245)?

   • If no, what are my out-of-network benefits?
